User Data Deletion Instructions

Effective Date: September 1, 2026 · Last Updated: September 2026 · Compliance: Meta Platform Policy & Philippine RA 10173

1. Overview & Meta Platform Policy Compliance

In accordance with Meta Platform Terms, the Meta Developer Policy, and Section 16 of Philippine Republic Act No. 10173 (Data Privacy Act of 2012), PatientCore OS ("PatientCore", registration pending in the Republic of the Philippines) provides this dedicated instruction page detailing how users can request the deletion, unlinking, or purging of their personal data and third-party app authorizations (including Meta, Facebook Login, Instagram, and WhatsApp Business integrations).

PatientCore respects your right to control your personal data. We provide two straightforward methods to remove or purge your data from our systems.

2. Method 1: Automated Data Removal via Meta / Facebook Settings

If you signed into a PatientCore service using Facebook Login, or authorized PatientCore through a Meta app integration, you can revoke access and trigger an automated data deletion request directly from your Facebook account:

STEP 1

Log in to your Facebook profile on web or mobile.

STEP 2

Click your profile picture in the top-right corner, select Settings & privacy, and click Settings.

STEP 3

In the left-hand sidebar menu, scroll down to the Permissions section and click Apps and Websites.

STEP 4

Locate PatientCore (or PatientCore Primary Care OS) in your active apps list and click Remove.

STEP 5

In the confirmation modal, check the box if you wish to delete past interactions and click Remove.

STEP 6

To trigger an automated server deletion callback: Click View Removed Apps and Websites, select PatientCore, and click Send Request. Facebook will automatically transmit an authenticated deletion callback payload to PatientCore's deletion endpoint.

3. Method 2: Direct Deletion Request via PatientCore DPO

You may also submit a direct request to our Data Protection Officer to permanently purge or anonymize your user account, contact information, and associated credentials:

How to Submit a Deletion Request:

Send an email to our Data Protection Officer at hello@patientcoreos.com with the subject line: Data Deletion Request - [Your Name].

Please provide the following details to verify your identity:

  • Your Full Legal Name
  • The email address or mobile number registered with PatientCore
  • Your Meta / Facebook User ID or profile link (if applicable)
  • Specific details of the account or interaction you wish to delete

4. Scope of Data Deletion & Processing Timeline

Upon receiving an authenticated deletion callback from Meta or a verified direct request from a data subject:

Statutory Medical Records Exemption (Philippine Law):

Please note that under the Data Privacy Act of 2012 (RA 10173, Section 4) and Department of Health (DOH) Administrative Orders, clinical diagnostic data, signed physician SOAP consultation notes, and official prescription ledgers are legally classified as medical records subject to mandatory statutory retention periods (up to 10 years for adult outpatient records). If you are a clinical patient whose records were documented by an accredited physician, clinical notes cannot be unlawfully erased prior to statutory expiration, but all external social logins, marketing contacts, and third-party app tokens are immediately and irreversibly purged.

5. Tracking Your Deletion Request Status

To check the status of a pending data deletion request, email our compliance team at hello@patientcoreos.com quoting your Deletion Confirmation Code. A final confirmation of completion will be transmitted to you once database purge routines finish.
